Albert Amoah bagged the winning goal for the home side in stoppage time after a defensive mix-up from Hearts of Oak.
Hearts of Oak center-back Kelvin Osei Asibey and goalkeeper Benjamin Asare got in each other's way, presenting a gaping goal to Amoah, who tucked home from close range.
Amoah's late winner sent the Baba Yara into a state of utter delirium as fans roared in celebration.
